I agree about the float lock on each side. Would have thought it would have been apparent right away. Would also check to see that both both flex levers are locked and the actual tabs are in the lock position. Sometimes when locking the flex the tabs can hang up and not go into their corresponding slots.
After that I would check the springs on each side of the adapter to make sure something didn’t break. Also I believe under the header there should be two white shear tabs that you should check and make sure they are not broken. They are positioned under header on each side where the header flexes. Kind of like a shear pin but shaped like a T.
